I'm a 31 year old female with under eye hollows/tear trough deformity. I've had them for as long as I can remember, but they have gotten a bit worse, most noticeably when I smile. I hate the deep groove/wrinkle that now appears when I smile now in photos. I've been considering getting under eye fillers for some time but am so scared about it. I'm getting married in a year and am wondering if I should do it now--would fillers improve the "groove" that I hate so much?
Answer: Under eye fillers I think you are correct to consider fillers if this is bothering you. Fillers under the eye can really make nice improvements by reducing the shadow that makes patients look tired. Blepharoplasty would be another option, but this is a surgical procedure that can be very tricky with eyes like yours. Surgery would likely have to involve fat transfer or relocation to fill in the groove. Fillers are much easier and can be reversed if you do not like the effect (assuming you get hyaluronic acid fillers such as Restylane). I have had good results with the relatively new Restylane Silk filler. You should avoid Juvederm under the eyes since it can cause the Tyndall effect, a bluish discoloration under the skin due to the way light refracts through the filler.

A common complication with this procedure is a blueish discoloration under the eyes. It is referred to as a tindle effect (or Raleigh scattering). This occurs because of shallow injections, the product used and the lack of experience.In my practice I prefer to use Belotero for tear troughs. Due to its irregular molecular size it has less tendency to cause discoloration, and has a very smooth result. Restylane and Juvederm are fine products as well.
