Since the steroid remains around for about three to four weeks following the intralesional steroid injection, it is not surprising that the indentation remains for this times and even deepens. The atrophy can at times spontaneously resolve over time without any treatment. I have successfully used intralesional saline injections with or without subcision to reverse this problem. Once a steroid such as Kenalog is injected, it continues to act for a period of three to as many as four weeks. At this point in time I would not attempt to do anything or it can actually harm you in more ways than imaginable. I would wait for an additional two weeks and see a plastic surgeon who specializes in fat grafting. I would also use hyaluronidase to dissolve most if not all of the hyaluronic acid filler which is clearly seen in the photographs in the lower eyelid area.
