Is it safe to have surgical hairline advancement while on low-dose isotretinoin (aka Accutane)?

I take long-term, low-dose isotretinoin (10mg per day) to control very oily skin. I know the conventional advice was to discontinue isotretinoin for several months before any elective procedure. I understand the research and recommendations have evolved and the old advice may be considered outdated. I'm not taking a short course for acne, but am on a long-term, low-dose regimen for oiliness. What would your recommendation be looking ahead to a planned SHA/forehead reduction?

This is between you and your surgeon, so follow their advice on this. I would suggest my patient hold the isotretinoin (even low dose) prior to an aesthetic procedure, especially forehead reduction given the visibility of the scar.

Hi Steven! Excellent question. There is quite a bit of controversy as you’ve mentioned regarding the timing of elective procedures on the face and isotretinoin usage. For low doses, some would make the argument that it shouldn’t effect the overall outcome of something like scalp advancement. I would have to defer to my Dermatology colleagues to ensure the safety of concurrent usage. My feeling is that it wouldn’t be a strict contraindication, but one where the risks would need to be discussed. Hope this helps!
