Revision or fat transfer for laxity and skin bunching? (Photo)

Question

Im 51 and has a lower bleph wirh a skin pinch , canthopexy and vertical mid face lift through the lower lid incisions. I see laxity and skin bunching when I smile that wasn’t there before the surgery. Do i need a revision or a fat transfer? I have very think skin and tried cool peel ans it thinned the skin under my enues prior to survery and didn’t tighten it.

Based on this photo, you are volume depleted; in other words,  you need some fat. This should include cheeks, lateral orbits and superior orbital rim/eyelids. This will require service of a very skilled surgeon with long experience with fat grafting. Good luck.

Fat transfers would not be effective.  You have excess skin of the upper and lower of the eyelids.  This can be corrected with an upper and lower blepharoplasty.

It doesn't look like you are smiling in the picture so it is tough to see the skin bunching you are referring to when you smile.. In general, bunching at the outer part of the eyes is best treated with a toxin like botox to relax the muscles which cause them. Lasers and peals are good at treating skin quality issues such as 'creapey' skin or very fine lines. The cool peel laser is not as strong as other lasers, so you may obtain better skin tightening with something else. Lastly, regarding fat transfer, the photo cuts off before showing the rest of your cheeks, however your " tear trough " does not seem pronounced
