Hi, I was wondering what kind of procedure would be suitable for my eyes. Eyelid surgery? fillers? or fat graft? As you can see my upper eyelids are droopy , Idon't know if it is ptosis. Also, my eyes are sunken and I have undereye hollowness. I have pufines under my eyes

It appears that you have a mild ptosis in both eyes. Surgical correction would help. Regarding the lower eyelids/cheek, it appears that a small amount of fat grafts would help improve this area. Based on your photo, here is a breakdown of what you might be seeing and how we typically address it:
Upper Eyelids (Droopiness vs. Ptosis):You definitely have some excess skin resting near your upper lash line, which creates that "heavy" or droopy appearance. An Upper Blepharoplasty is the gold standard procedure to carefully remove this excess skin and open up the eyes. You asked a great question about ptosis. True ptosis means the actual muscle that lifts the eyelid (levator muscle) is weak or stretched. While it is difficult to definitively diagnose true ptosis from a single static photo, if an examination shows your muscle needs tightening, a Ptosis Repair can easily and safely be combined with your upper blepharoplasty during the same surgery.
Lower Eyelids (Hollowness and Puffiness):The puffiness you are noticing is likely a small amount of natural orbital fat pushing forward. This becomes much more noticeable when there is a lack of volume (the hollowness or sunken look) right below it.
The Surgical Route: A Lower Blepharoplasty with fat repositioning, or combining it with Facial Fat Grafting, provides the most permanent and natural-looking solution. Fat grafting is particularly excellent for sunken eyes because it restores soft, long-lasting volume using your body's own tissue.
