So-called "accordion lines," also known as parallel laugh lines are a fairly common problem that I have been treating with gratifying results for many years. It is usually quite easily treated by the appropriate choice of a low viscosity, malleable, cohesive HA filler injected in the proper fanning technique to provide the necessary support below the skin to resist the muscular action responsible for the appearance of these lines with smiling. The results of treatment are typically immediate and treatment usually engenders little to no signficant downtime. You would be wise to consult with a board certified cosmetic dermatologist filler expert and be sure to ask to see his/her before and after photos of the treatment of this problem. You are asking early on before it has become severe which puts you in a much better position to prevent it from worsening. There are a variety of approaches to manage skin texture and fine lines that you are describing. For a dramatic result, you would ideally undergo laser resurfacing with an ablative laser, such as erbium-yag or CO2. You can also have similarly dramatic results with a phenol peel. In addition to resurfacing, you should make it a regular part of your skin care routine to use sunscreen every single day and avoid any sun exposure to your face if possible. Most of the time, when patients form an accordion on their face, while smiling, it is due to loss of facial volume. It is a natural process of aging. In your best interest I would recommend an in person consultation with the Plastic surgeon or at least supply another set of photographs, which also show you smiling. Having the whole face exposed with the eyes blacked out would be the best way to go about it. I gave you my answer above based on my experience without the appropriate photographs.
