I am 5”4 and 145 pounds. i have had sagging breasts since i hit puberty at 9. i also lost some weight around the age of 14, which im guessing contributes, but my breasts have been saggy since i was about 11, but that’s not something you really think about at that age. i hide it from all of my friends, but i am genuinely extremely insecure. i am unable to wear bathing suits, most normal bras cause extreme gapping, and even tube tops have to be paired with a bra. i have a lot of friends and we have all seen each other naked at some point, and even my friends with bigger breasts don’t sag this much. i am also decently active, i work out my chest muscles pretty often. i’m not sure if a breast augmentation is even worth it, because all of the skin that is there is probably what makes my boobs have any volume, and i don’t like the risks associated with implants (or the aesthetic of fake boobs). i have thought about surgeries since i was at least 14. i absolutely hate my breasts, and the only thing i have not done is ask a professional.

Answer: Treatment options To improve the appearance of your breast, you have basically two options. One is to have just a breast lift. The other is to have a breast lift with implants. The drawback to doing only a breast lift is that the breast has a tendency to look flat and lack volume in the upper half of the breast. Adding the implant gives projection and volume in the upper half of the breast. Regardless, you’ll need a breast lift if you want better looking breast. There’s also the option of fat transfer. He would still need a lift but fat transfer can add about a 1/2 cup increase in breast size. That transfer is not a substitute for an implant. Fat transfer does not change the shape of the breast in any significant way, but can make what you have a little bit bigger. You would still need a lift regardless. Patience who have skin that tends to stretch out may continue to stretch after having a breast lift. The skin envelope may need to be re-tightened with a revision procedure down the road after breast lift surgery. I suggest having a few in person to consultations with plastic surgeons in your community. In your case, I would not opt for a short scar lift. You want a lift that’s going to do the job right. This most likely means having an anchor type scar. Schedule several in person consultations with plastic surgeons in your community who seem to have a lot of experience with breast surgery. Bring pictures of yourself to use as reference during each consultation. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar breast characteristics. Preferably patient of the same age who have not had pregnancies. Take careful note regarding the quality and quantity of before and after pictures. Experienced surgeons should have lots and lots of before and after pictures for you to look at. Preferably at least 10 to 20 patients. You may not need to see that many results but they should be available to confirm the providers experience level. Ask providers what the most common indication for revision surgery is, what their revision rate is and what their revision policy is for this procedure. You can definitely have a lift and then add implants later if you desire. Best, Mats Hagstrom MD
