The expert use of an HA filler can work well for elevating and smoothing under-eye hollows and creating a smooth transition between the upper cheeks and the under-eyes. If still necessary, for additional skin lightening, a series of in-office superficial peels can be performed. Be very cautious when considering fat grafting. It's an expensive two-step, minimally invasive process whereby the fat is removed from donor site, such as the belly, hips or buttocks, and then processed and injected into the recipient site. Since large bore cannulas are needed for both processes, there is typically a prolonged post-treatment downtime. What's more, once separated from its original blood supply, much of the transplanted fat does not survive. And finally, keep in mind that the fat that does survive retains the characteristics of the area from which it was taken. This means, for example, if the fat was taken from belly when in time you develop a midriff bulge from age, the fat transplanted under the eyes can do so, as well. Bottom line: be cautious when considering fat grafting. If additional lightening is required under the eyes, consider a series of in-office superficial peels coupled with the at-home use of prescription skin lightening topicals.
