Previous Breast reduction gone wrong, now what...any suggestions for a revision? (Photos)

I had a breast reduction in 2012; which all incisions opened, I had roughly 7 months of wound care, infection etc. I was terrified to seek any further care after all was said and done however now my boobs are so misshaped, saggy with horrible scars. Any recommendations... without out pocket lift / implants. Financially that is not in the cards

Your photos suggest that you should achieve substantial aesthetic improvement with a revision of the breast reductions. This could provide a lift to your breasts, improvement in the scars and additional volume reduction.

Dear Leilaan: So sorry to see the results of your previous reduction. A secondary mastopexy could be done but with the following caveats. 
You should be totally nicotine/smoke free (even second hand smoke) .
The surgeon would be best served by the having access to the previous operative records (to see which blood supply was left intact) .
You should realize that new size is limited to your own breast tissue volume (B/C ish) if you don't us an implant.
And finally, it may not be possible to remove all of the scar formation below the areolas. 
You should schedule post surgical hyperbaric treatments pre-op (hoping to cancel them post-op). It's far easier to cancel if not needed than to add on if needed urgently. 
The physical pain/discomfort will be minimal. But if you travel to an out to town surgeon, you should be prepared to stay in town 10 days or so after surgery.

You had a rough time with recovery from that surgery... The good news is that time has been your friend and there now appears to be substantial laxity in the skin which should permit significant improvement in the cosmetic appearance. A great deal of or potentially even all of the bad scarring can be removed to give the body a new opportunity to form prettier scars. It will be very important to get your previous operative note for your next surgeon and also to optimize any medical issues that could impair wound healing. Examples include diabetes, nicotine use, and immunosuppressant medications. Good luck!

I’m sorry for the trouble you have had. It appears that your breasts were left too large which contributed to the incision breakdown.. they can be revised but much planning and caution is needed to preserve the circulation to the nipples because they need to be raised as well. Getting an implant at the same time would be risky and not advisable. I have seen the comment from Dr. Horndeski about his technique and although I have never done it, you would be the perfect case because there would be no vertical segment to breakdown as before.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery.

breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.

I'm sorry to hear of the issues that you experienced during your previous surgery.  Unfortunately the only way to improve the current appearance of the breasts is through a revision.  The breasts are different sizes and the nipple position needs to be lifted.  Special care must be taken to preserve blood supply.  I encourage you to visit with a few board certified plastic surgeons near you for consultations.
