I had bad liposuction in 2018. Before any doctor comments about how I need to do my research, pick someone with a good reputation, etc., I did all that. He is a well renowned surgeon in Nashville, TN with plenty of accolades, but he ruined my body . I have had two rounds of fat transfers and now I am out of fat. The last round was over 3 years ago. Is there anything that can be done to smooth it out so that I look less deformed?

In the photos, the thighs show visible contour irregularity with areas of waviness, dents, and some skin looseness, especially around the inner/lower thighs and knee region. This type of problem is usually not just “extra fat”; it can come from over-resection, scar tethering, uneven fat thickness, and reduced skin elasticity.

After two fat transfer revisions and limited donor fat, further correction becomes more difficult, but there may still be options. A revision body-contouring surgeon would need to examine whether the main issue is tethered scar, volume deficiency, skin laxity, or remaining uneven fat. Depending on the exam, treatment could include careful scar release/subcision, very conservative touch-up liposuction of surrounding high spots, small-volume fat grafting if any donor fat remains, or selected fillers/biostimulatory injectables in limited areas. Energy-based skin tightening or microneedling can sometimes improve mild laxity or texture, but they usually do not erase deeper dents.

The most important point is to avoid another aggressive liposuction procedure, because that can make over-resected areas worse. I would seek an in-person consultation with a board-certified plastic surgeon who specifically does secondary liposuction contour correction, and go in expecting improvement rather than perfection.

Hello; My suggestion to you is; to have PLA injections in large areas, between 3-5 sessions. This; will both fill the hollow areas and since it stimulates for collagen formation; it will also create for tightening in the skin. In order to increase for the affects of PLA, I recommend for having it done; in Thermage Body; 1-2 months after the relevant injection as well.

I understand your concern. I would have to examine you but it appears that this is a contour irregularity after liposuction. This happens often when areas the undergo liposuction are not feathered appropriately.
The good news is that it can be fixed and I have done so with good results before. While there is always a chance for contour irregularities with liposuction, someone who takes their time and uses the appropriate sized cannulas should give you a superb result.I have all my patients wear compression garments and I use multiple sized cannulas to feather out the result.However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

you might try either BodyTite or Renuvion to help smooth the skin.  BodyTite uses Radiofrequency while Renuvion is a plasma based device.  Both generate heat which causes tissue contraction which has a smoothing effect on the skin

I would suggest Sculptra injections, which can help rebuild thin areas. It will be a few sessions and build gradually. Find someone who does a lot of Sculptra, especially in scarred areas.

More information is needed for a specific recommendation, but you may want to consider options to tighten the skin.  BodyTite is a minimally invasive procedure that does not require general anesthesia.  It uses RF technology to help retract the skin.  Another option is J Plasma.
