Nasal spine reduction can be performed through a internal columellar incision. The nasal spine is exposed and can be infractured with an osteotome or completely removed. In general, the nasal spine does not need to be completed resected. In most rhinoplasties, the nasal spine is not addressed. Nasal spine reduction can be done from an incision inside the nose or from under the lip. It involves removing the protruding nasal spine bone and sometimes a bit of the septum attached to it.
Reducing the nasal spine is also known as a columellar-plasty. This involves shaving both bone and cartilage from the nasal spine, columella and trimming back skin on both sides of the columella as well. There is no reason to perform eyelid surgery, so leave well enough alone. If you decide to do a rhinoplasty, be very specific on the goals you are attempting to achieve on the nose (not the eyelids)which would be minimal at best.
HI,I would say that ultimately you will need an alar rim graft to correct this asymmetry. I dont think any stitching will suffice. You should wait 2 to 3 months before doing anything but I dont think anything will change.Best,Dr.S.
