, Hypomastia is fairly common in young men going through puberty. It has nothing to do with masturbation. Stimulation of the breasts by friction from clothing over a length of time can cause the breast tissue to swell and cause discomfort.

You should see a plastic surgeon for an exam. If you have extra breast tissue under the nipple, then surgery may be the only solution, but some meds might help.

To me, it looks like you have mild gynecomastia.  You didn't mention your age, however you look to be in your teens or twenties.  You are in good shape, and much of the time, this will resolve all by itself.  Sometimes, medications, and illicit drugs may cause this condition, and cessation may help the situation to regress.

If things don't go away all by themselves, liposuction would be a good option.

Yours is an example of areolar gynecomastia. Surgery is the only solution which is done by direct excision through an areolar incision.

 

What you have is gynecomastia. Unless there is some underlying cause like a drug you are taking or an endocrine imbalance, the only solution is surgery. What kind of surgery depends on what the excess tissue is, breast tissue, fat or both. Occasionally, not even correcting the underlying problem resolves the gynecomastia and you will need surgery to flatten the area. Your first step is to see your family doctor and determine if there is an underlying cause.
