From your pictures, it appears that you're very early in the post-operative period of a breast lift with weight reduction. As this surgery typically has a lot of swelling, it often takes 3 to 4 months to see the final results.  Try to be patient and allow the results to take place.

You need to be in close contact with your plastic surgeon to manage this potentially serious issue. Early intervention is the best way to correct problems. 

Taking a quick look at your pictures it seems like your surgery was recent so you should consider to adress your doubts with surgeon as he is the one who can explain you if there is any issue with your breast, so please be patient and dont consider a revision until you see your finals result which will be seen after 4-6 months after your surgery. It sounds like you had a breast lift with reduction of volume to have gone from a DDD to a D. It has been my experience that women feel their breasts are smaller once the excess skin is removed even if no breast tissue is removed with the skin. It also looks like from your photo that you are still healing, so I am assuming you are not more than a few weeks out from your surgery. Be patient the shape of breasts after a lift or reduction can be a little funny in the beginning. Depending on the technique used, they can appear boxy. They will settle over 2-3 months after surgery and the shape will become more natural.
