Usually the side that requires more "work" by the surgeon is the one that tends to swell more.Buccal fat removal post surgical swelling could last several weeks in some patients (e.g 5 weeks or more). Healing post buccal fat removal takes up to three months. Therefore, residual assymetric swelling before 3 months could be normal in some patients.
Swelling varies from patient to patient. It also depends on how much dissection through the buccinator muscle had to be done in order to reach the fat pad - this dissection can influence significantly the amount of swelling, and can be a simple explanation for asymmetrical swelling. Truth is, I recommend giving your operating surgeon a call and asking him what to expect as they performed the procedure and will help guide you through the post operative process.
