Question

Hello, I have been on Accutane for around 12 weeks now. Accutane was primarily for my chest and back not as much for my face. When I began to get the "breakout" everyone seems to get while on accutane, my jawline broke out quite badly and has never seemed to heal. My jawline is quite red and has been since around week 3. Also My back and chest have gotten little improvements... The first month was 40mg/day and then then next two months have been 80mg/day. I am 17 years old and weigh about 155 lb

While accutane (isotretinoin) is a terrific medication, there are some side effects that can occur.  This worsening can occur occasionally and is a normal reaction.  Sometime, however, it can become worse than expected and require medications by your dermatologist to get it under control, such as prednisone.  You should speak with your dermatologist about it at your next visit.  Good luck!

Accutane is a go-to medication for patients with recalcitrant cystic acne. It can reduce pore size and also reduce sebum production in your skin, making it less oily; and in turn, dryer. Accutane can be used to control acne, but patients receiving the medication should be carefully monitored. In terms of skincare, always wear an appropriate sunscreen and reduce direct sun exposure. In my patients, I recommend a good ceramide moisturizer such as Elevase whenever they are on Accutane. This will keep the skin hydrated and reduce the oiliness of the skin that is associated with oil-based moisturizers.

There are several possible reasons for the acne flare. You should be consulting with your doctor. The dosage you are on is appropriate for your weight, however if your acne is not clearing up  after 3 months, a small dosage increase or addition of an oral antibiotic that does not conflict with Accutane are both options. Let your doctor know about any supplements you are taking. Some muscle boosting supplements can increase testosterone which will flare up acne
