Possible eyelid surgery - how should I proceed? (Photo)

Question

I have this issue with my eyelids for years now. I do remember they did not always look this way. Which side is normal? Why are they like this? Is it just swelling? I’m not really sure what doctor to go see first as well or if I just go straight to a surgeon. Any insight would be so helpful thank you in advance. I just feel like a monster and this is my biggest insecurity

Great question and thank you for providing a photo!
It looks like you have both eyelid and brow asymmetry, though I think the eyelid asymmetry is a bit more noticeable. It is hard to tell from the photo what the exact cause is for this, but what I see is a bit of excess skin of both upper eyelids. I think the skin is just folded differently on either side which is giving the asymmetric appearance. My initial recommendation would be to see a plastic or oculoplastic surgeon, ideally someone who specializes in eyelid surgery, but I do feel you would likely be a candidate for upper blepharoplasty to help with your asymmetry.

Thank you for your photo. I believe you should first have your eyelids checked for ptosis to be sure there isn’t an underlying abnormality with your levator muscle. If your muscle insertion and excursion is normal, it appears that an upper blephroplasty would allow you to achieve more symmetry of your upper eyelid skin and eyelid crease.  A plastic surgeon should be able to help you and direct you to the best options to correct this. Best wishes.

Based on this photo you may have a droopy eyelids, condition called upper eyelid ptosis. Treatment of this condition is surgical. In person consultation with a board certified plastic or oculoplastic surgeon is recommended. Good luck.
