What caused extreme hyperpigmentation / skin discoloration after thigh lift with lipo and 360 tummy tuck with lipo?

I’m 5’2 blk & had lost over 160+ pounds when I went to Dr.Miami. I weighed 170 surgery day & after surgery I have two skin tones from where he lipo / cut for the thigh lift and tummy tuck specially my vagina & all the way down my inner legs.It’s been 3years. What causes this?can I fix it? Was this negligence?is this something that shouldve been explained as I’ve asked & flown back 4x times to ask in person too because I’m so insecure.
