Basolabial angle and flat upper lip - could rhinoplasty address this? (Photo)

Question

I am unhappy with the look of my nasolabial angle and the flatness of my upper lip. I habe just completed teeth alignment with invisalign and have had filler for projection in my upper lip, albeit a conservative amount. Is this something a rhinoplasty could address?

The profile photo shows a relatively flat upper-lip projection and a defined transition beneath the columella, although the drawn line and a single nonstandard image limit measurement of the true nasolabial angle. Rhinoplasty can alter this angle by changing tip rotation, caudal septal length, columellar position, or tip support. It may soften the nose-to-lip transition if one of those nasal structures is responsible. It will not directly advance a retrusive upper jaw, move the teeth, or reliably add projection to the red lip. Those possibilities require separate assessment of the bite, maxilla, tooth support, lip length, and movement when smiling. Over-rotating the tip simply to open the angle can increase nostril show and create an unnatural result. Because Invisalign and filler have already changed the surrounding anatomy, allow the filler to settle or dissolve if necessary before definitive measurements. A rhinoplasty surgeon and, when skeletal or dental support is uncertain, an orthodontic or maxillofacial specialist should evaluate the profile together before deciding whether the correct treatment is nasal, lip-based, dental, skeletal, or no surgery.

Based on your isolated profile photo, we can see excessive nostril show, and a tip that sticks out a drop too far. Rhinoplasty surgery may be considered for achieving tip deprojection, rotation, and likely a drop of bridge lowering, for a less distracting, and more feminine nasal appearance. I hope this helps! Maneuvers for your surgery would include dorsal hump reduction, tip refinement, nasal tip deprojection, tip elevation, among other maneuvers. Based on your photo, rhinoplasty can help improve the nasolabial angle by adjusting the position and rotation of the nasal tip. However, it will not significantly address the flatness of the upper lip, which is typically better treated with lip-focused procedures such as fillers or a lip lift.
