Hello RealSelf community, thank you in advance for your time in reading this. I ask for your expertise and advice in a cosmetic solution for the following so I can proceed with information. (I am a 27 y/o male) 1- Uneven spot in lower cheek area / lower sides of mouth. Very visible in photos and in person, really want to get rid of these 'bump's. 2- Small and bumpy chin. (I believe this can be fixed with a chin implant to give it more length/definition, would like any advice) Thanks again.
Answer: Chin and Perioral Mounds Your circled areas are perioral mounds which are treated by small cannula lkposuction from an incision inside the mouth corners. The dimpling on your chin is caused by a short chin and a mentalis strain. Whether that should be treated by a chin implant or a sliding genioplasty can be debated. But the combination of a vertically short chin with mentalis strain favors the bony movement.

Answer: Midfacial descent So mid 30s is the time when this sign of ageing first appears. As the mid face deflates so the soft tissues fall. As the fall they start to weigh around the fix points at the corner of the mouth and the lower jaw. You have two options when you are young. The first and better option for you is to reinflate the cheek. This can be done with fat or filler and will correct these early signs. In ten years time the tissues may have fallen so far that lifting might be needed but I think that option should be left in your case for the future. Hope that helps. Adam Goodwin

Answer: Volume correction The problem near the mouth corner typically occurs as volume is lost from the midface and the skin begins to slide forward and down. I would recommend revolumizing the cheeks with any filler you like (Voluma, Bellafill, Sculptra, fat, etc) to help lift that skin and minimize the lower face "bumps." As far as the chin goes, an implant would be a nice way to give you more structure and strengthen the look. For the dimpling you could consider Botox. This treatment is simple but will need to be repeated regularly for sustained correction.

Hi, I have performed many facial shaping procedures using dermal fillers, silastic facial implants (cheek implants, chin implants, lip implants), liposuction and/or facelifts for over 30 years. From the photos, your chin is very weak. I have performed many Chin Augmentations using chin implants for over 30 years. A weak chin creates an imbalance making the nose appear larger, the mid face top heavy and the lower face look short that de-emphasizes the lips and allows early formation of a double chin and "pseudo-jowls" (the "bumps" hat you are referring to in your post). Pseudo jowls differ from "jowls" in what causes them, what they represent and how they are best treated. Pseudo jowls are defined by a fold of tissue laxity that goes from the corners of the mouth to approximately 1/3 or 1/2 the distance to the lower jaw line. It’s caused by a weak chin and its lack of tissue support around the chin and mouth area. It's best treated by correcting of the weak chin with a silastic chin implant. You do not have "jowls", nor would they be expected in a young face such as yours but they are best treated with a SMAS facelift. I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty. I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone. In my opinion, you are a good candidate for chin implant surgery. The cobblestone (bumpy surface) over the chin is the result of an over active Mentalis muscle that is best treated with injections of Botox or a similar neuromodulator to selectively weaken the muscle.
