I will be getting a chin implant. My surgeon is extremely experienced. He intends to have me use a chin strap after the surgery. Will the chin strap dislodge my chin implant? Also, will my silicone chin implant cause bone erosion, or is bone erosion a theoretical risk? Or is bone absorption a risk? My surgeon doesn't intend to use screws to secure the chin implant. Will my chin implant then be at a greater risk of being crooked?

Answer: Migration of a chin implant The implant is placed in a conservative pocket and most surgeons suture them in place or place a small bone screw. I prefer a small bone screw. The chin strap will not cause migration of your chin implant. It is possible for an implant to migrate and the issue of bone resorption is rare. The most common complication with implants is infection.

In my practice, I do not use fixation screws because I feel they aren't necessary; other Surgeons will have a legitimate difference of opinion. Because of the factors mentioned above, a chin strap should not lead to malposition of your implant. Bone resorption is noted with silastic implants, but is generally not clinically significant, happening slowly for years after implant placement.

Best to check with your operative surgeon. In our practice, we do not use chin straps or screw fixation. Bone does not absorb, it just remodels and it is never been an issue since the mandible is approximately 1/2 inch thick in that location. Chin implants are manufactured in a large array of both sizes and shapes, are composed of solid silicone, and can be custom carved when necessary to match patient's unique anatomy.
