Do I need chin implant and chin lipo with facial fat transfer? (Photos)

Question

Please provide professional and honest advice on what would benefit me. I was considering a chin implant with chin lipo. My face also seems like it needs fat transfer to certain places? Buccal fat removal? I am unsure. Please evaluate and help me figure it all out...

Thank you for your question and photographs.

As a starting point, I would suggest that you try to evaluate what it is you wish to achieve as a final shape for your features, both for the mid-face, the chin, and the neck.

Working simply on the basis that if there is a problem, it should be fixed in comparison with the average contour or ideal contours does not necessarily generate the most satisfactory outcomes.

The images that you have uploaded show volume deficiency in the mid-face, but in my opinion they do not show any significant amount of buccal fat prominence. Volume augmentation in the mid-face can definitely be a balancing effect, and I honestly don't see the need for the buccal fat reduction. For the contour of the jawline, chin, and neck, there seems to be a degree of underdevelopment of the lower jaw combined with a hereditary volume excess under the chin. The chin augmentation can address this, either with an implant or genioplasty.

The contour under the chin depends to some degree on your current BMI.

If your BMI is low, there may not be a significant amount of fatty tissue to reduce under the chin, and attempts to overcorrect can lead to irregular contours for the neck skin. The anterior neck lift intervention is typically positioned to address the deeper-seated volume and would, of course, be able to impact fatty tissue volume under the chin situated in the superficial layer under the skin.

My suggestion to you would be to start with 3D imaging and computer simulations for the appearance of your face to allow you to determine the characteristics of the shape change that you would feel most comfortable with. Based on that, to identify treatments that would be compatible with your expectations.

The amount of horizontal projection that you need in order to balance your lower face is significant.  When female patients require greater than 5 mm of horizontal advancement, I prefer performing a sliding genioplasty. A chin implant that is greater than 5 mm can sometimes make the chin appear wide and masculine. I think that you will need 7-8 mm of chin advancement. Fat transfer is also a good option for places in the mid-face that need volume. This can all be performed simultaneously in the office with sedation anesthesia.

You appear to be a good candidate for a chin implant. You may need liposuction but that can only be determined by a physical examination, not a photograph. I do not recommend buccal fat removal as that can lead to hollow and is almost impossible to correct.
