I can not speak for Mentor. All implant manufacturers offer warranty coverage for breast implants. They typically replace the implants if they break. Some will also replace implants in the face of capsular contracture. The time after surgery is also limited. Additional coverage may also pay for expenses such as outpatient facilities, anesthesia, and surgeons fees. The depth and scope of coverage depends on the individual company. Some implant companies offer additional coverage through third party insurance carriers. So if you want a comprehensive explanation of coverage, I would contact the manufacturer. Do I think that supplemental coverage is a good idea, Yes! You might feel that it's a waste of money if everything goes well, but it's a very good investment if you have problems. Is pain a covered problem, that all depends. If it's due to breakage or capsular contracture, you might have a good argument. But as in all things, it depends on the details.

To the best of my knowledge, this warranty does not cover pain following surgery. It would be best to research this on the mentor website. The warranty is for breakage and in some cases also for treatment of capsular contracture. If one is having discomfort a number of months following surgery, it would be wise to pay a visit to your surgeon who may be able to help figure out the causation of your discomfort. There may be therapies that can help.
Mentor's warranty depends upon what kind of implant you have--gel or saline, and if you purchased the additional warranty for gel implants within 45 days from surgery. If you purchased the additional warranty for gel implants and have a capsular contracture of grade 3 or 4 that is causing pain, this should be covered. Otherwise, just having pain is not covered by the warranty. Please discuss your warranty with your surgeon.
