I am obese. It seems like it’s a bunch of fat on the right side of my butt my butt crack more to the left at the top. Is this simply just fat and can be fixed with lipo/weight loss? or could it be a spinal issue?

Answer: Asymmetrical gluteal crease. During every embryological development, the posterior midline goes through a period of fusion from the two sides of your body which initially develop independently, and eventually fuse in the midline. Lower spinal area is known to have potential congenital defect, which typically happen during embryological development at the time of midline fusion. I don’t think the sloping of your gluteal crease is related to fat distribution so Liposuction is probably not going to be best treatment. Most likely this area is best left alone. You could probably have liposuction to flatten the area out but the deviation to the left is going to be challenging to correct. Best, Mats Hagstrom, MD
