Please Help - Advice on Revision Rhinoplasty (Photo)

I would find a plastic surgeon certified by the American Board of Plastic Surgery. You need an in-person evaluation and physical exam. I would recommend a revision rhinoplasty. You cannot simply remove the septal extension graft....there has to be some type of tip support. An option would be to contour the septal extension graft much better or perform a columellar strut. Regardless, revisionary rhinoplasty surgery is incredibly complex and requires a true expert. I always tell prospective patients to look at the plastic surgeon's training (ideally a 6 year integrated plastic surgery residency and a 1 year aesthetic fellowship accredited by The Aesthetic Society). In addition, examine before/after photos on social media platforms. I would also look at Google or RealSelf reviews. This will help you get a comprehensive understanding of your provider.

Hi! You are still healing if you are 10mo out. The SEG is probably contributing to the feeling that your nose is hard but so is swelling; if you remove the SEG, there's no guarantee that it won't continue to feel hard. For the definition, I see your concerns but with you only being 10mo out, I would still recommend waiting for more of that to resolve and/or considering steroid injections to get that process going faster. The redness/thickened skin combo suggests to me that maybe you have rosacea you might want to talk to a derm about? I would be cautious about removing the SEG unless your surgeon says it would be safe to do so because almost always it was put in there for a reason (looking at your preop photos, my guess is it was put in there to help straighten the tip and probably add some tip support). If you take the SEG out that you will lose that.
