Here’s what’s likely happening:
After the initial 5 months of growth, your scalp entered a synchronization phase, where both transplanted and native follicles temporarily shed due to hormonal fluctuations and cycle reset.
This process can be triggered when supportive treatments like PRP and LLLT stop, as those therapies help maintain scalp circulation and growth factor stimulation.

The good news: transplanted grafts are permanent and will regrow. The thinning you’re seeing now is temporary, and density will usually recover over the next 2–4 months once the follicles re-enter the anagen (growth) phase.

However, at 18, your native hair is still highly DHT-sensitive, meaning ongoing medical therapy is essential to stabilize AGA.
I recommend:

Starting or continuing finasteride (under doctor supervision) or topical anti-DHT therapy,

Continuing PRP/LLLT maintenance every 2–3 months,

I do not recommend hair transplants at this young age because the hair loss rate is at its peak. There will be a high hair shock loss from doing surgery at a young age; which you are experiencing right now. At this moment, I recommend that you take Finasteride and apply Rogaine to slow down the hair loss. Doing PRP with multiple shots on the scalp can cause more trauma and scarring which can lead to further shock loss at this stage. It is best to leave the scalp alone and just take Finasteride and apply Rogaine.
