What you’re describing sounds like a late-phase shedding, which is quite common between months 8–10, especially in younger patients or those with ongoing androgenetic alopecia (AGA).

There are two main possibilities here:

1- Shock loss of native (non-transplanted) hair:
Even after a successful transplant, the surrounding native hairs can enter a telogen effluvium phase due to hormonal fluctuation, stress, or seasonal factors.
This shedding gives the impression that the transplant is failing — but the transplanted grafts remain intact beneath the scalp.

2- Progressive hair loss (untreated AGA):
If finasteride or another DHT-blocking treatment wasn’t used, your natural hair might have continued thinning while the transplanted hair stayed stable, creating visible density loss.

The good news:

The transplanted grafts are permanent and do not fall out permanently from such shedding.

Hair lost from shock or stress-related shedding usually regrows within 2–4 months.

Starting or resuming finasteride, minoxidil, and PRP sessions can help accelerate regrowth and stabilize native hair.

You’re still within the normal timeline for stabilization — with proper medical support, your density should recover gradually over the next few months.
