I love how natural breasts look when you lay down - would gel implants change the way they look when laying down?

Gel implants are not identical to breast tissue and do not move the same. At size 32, each 100 cc of implant corresponds to 1 cup size change. An alternative to implants is fat transfers. They are usually good for 1-1 1/2 cup size change in one session.

Fat transfer is also widely used in reconstruction of the breast after a mastectomy or lumpectomy. For the size you want, you will need implants. If you do not choose implants that are too large and heavy for your frame and tissue, you should be able to get the natural look. A DD might be somewhat large. For a starting B cup, I would suggest placing the implants behind the muscle as well.

Thank you for your questions. Certainly the larger the implant you use, no matter what fill (saline or silicone) will result in an appearance that is less natural. If you use a smooth round implant, you will have more upper pole fullness than you currently have, less when you are upright with more of the volume moving to the lower pole. When you lie down, this will distribute more naturally than an anatomic or tear drop shaped implant. The smaller the implant, the more of your own tissue you will have to camouflage the implant and the more natural they will appear. Other options for breast volume enhancement include use of fat grafting. The advantage of this is it is not a foreign body, thus is more natural by definition. For some women, and you might be one of these, you may not have sufficient fat to get the volume enhancement you are looking for. I would encourage you to seek out a board certified plastic surgeon to consult with for a complete history and physical exam so that he or she might be able to offer their expert opinion based on a more complete set of information.

Implants will definitely project more than native breast tissue when laying down. They just can not relax in the same way that glandular breast tissue will, by nature of its design. If that is a major concern for you, I would seriously consider autologous breast augmentation with fat grafting instead of implant-based augmentation. It can get you the volume you want and your results will move and respond to different positions similar to your native breast tissue.

It is most important that you directly discuss your concerns about implant shapes and chest wall positions with your plastic surgeon before surgery. Your breast implant appearances are highly dependent upon your pre-op tissue laxity, breast shapes and positions. If your tissue stretches adequately after enlargements and you do not have significant implant capsules, they may settle laterally when you are supine.
