6 mths PO - what can be done for uneven cleavage after bilateral submuscular breast aug & donut lift? (Photos)

Your preop photos show a variant of Tuberous Breast deformity in at least the right breast, with lack of skin and fullness below the nipple. Considering this, you have had a nice result from the augment and donut lift. However, your right breast does show more fulness superiorly and the lower breast border is just slightly higher than the left. The only solution I know about is to surgically lower the inframammary fold a little so the implant drift downward. Occasionally, ultrasound treatments have relaxed the scar tissue surrounding the implant, which can reduce superior rounded fullness if that is due to any tightening of scar tissue.

It looks like the asymmetry to begin with was then transferred to the final result. I would say either changing out the implant for a high profile or adding fat grafts to the breast would be the best bet to try and create symmetry. I would also talk your board-certified plastic surgeon about your concerns.

Like your surgeon, I am not a big fan of lowering the fold because some support may be lost and you are at risk of bottoming out with time. However, sometimes it should be done and extra support can be added. Please discuss this with your surgeon.
