Hello! Thank you for your question. As far as I can see from your photo inside of your nostril the bump might be a dislocated cartilage and you need to talk about it with your surgeon. Because it can only fix bu revision surgery. If you can provide photos with better lighting, from the side of your face and 1 meter away I would be able to make a proper evaluation. I’m frankly surprised that the surgeon did not have an in-depth discussion with you beforehand about the goals of the surgery and your expectations. Though it is true that spreader grafts placements alone had nothing to do with your hip shape, it will however widen your middle vault as seen from the frontal view. This will either be a good thing esthetically or not so. The main reason for placing spreader grafts is to prevent collapse of your deeper airways with inspiration which would correct it’s commonly associated inverted « V-deformity ».

For your surgeon to go to the added time, effort and expense of performing a tip plasty on you when you didn’t ask for it is puzzling in this day and age. This reflects to me more of a lack of communication for which I would advise you to voice your concerns with your original surgeon if you have not done so already.

Otherwise, seek the advice of an experienced board certified plastic surgeon.
