Hi there,Thank you for the question and photographs. You appear to have - as I myself have - prominent eyes, and it would be helpful to also see side views (a.k.a.,, lateral views) of your eyes. A canthopexy - in the right patient - can help establish better support to the lower eyelids, and when done well, can help reshape the eyes into a more feminine almond shape with a canthal tilt (angle between the inner corner of the eye and outer corner of the eye). I think what might be bugging you is the the lateral scleral triangle - that 'white' of the outer part of your eye, to the side of your colored iris - is disproportionately larger than your medial (inner) scleral triangle. I see some rounding along the lateral edge of your lower eyelid, which a well-done lateral canthopexy can address.However, this has to be weighed against relative contraindications, such as thyroid disease or a prominent eye - the latter refers to the extent to which your eyeball protrudes or bulges from the eye socket. A front-on view of your face and eyes does not show that as well as side views, and more importantly, a physical examination. A Hertel exophthalmometer is a formal instrument that can measure eye prominence as well, which most any well-trained oculoplastic surgeon has at his or her office. Doing a canthopexy can potentially be disastrous in someone with prominent eyes; if not done precisely, the tightening of the canthopexy can 'clothesline' the lower eyeline and exacerbate outward bulging of the eyeball (globe).Regarding your brows, I see the asymmetry you are seeing, and I wonder whether you have tried or exhausted Botox (botulinum toxin) first, to attempt to address the asymmetry non-surgically. While Botox lasts 3-4 months in most individuals, I would suggest that first before jumping into surgery, with someone whose face is as youthful as yours (your general age and station in life are helpful details!). Even something as simple as threading your brows differently might create a subtle improvement that could spare you the incisions and scarring of surgery.Failing those things, then a browlift may be helpful, whether an endoscopic browlift (Endobrow), browlift done at the edge of the hairbearing brow (supraciliary browlift), or browlift that can be done through an upper blepharoplasty (eyelid lift) access. I generally do not suggest these surgical options until someone is at least middle aged (40s on up), as the risk for visible scarring may outweigh your goal to achieve better brow symmetry. While an EndoBrow is great for many reasons - primarily hiding scars in the hair-bearing scalp, it has a tendency to elevate the medial brows more than the lateral brows, which can risk ending up with a forlorn look postoperatively, meaning the nice feminine arch to your eyebrows, with the lateral aspect higher than the medial aspect, can become reversed.A small supraciliary browlift could help elevate that brow (I am assuming it's your Left, but 'selfie' view with your phone can flipflop the images), but there is the risk of the scar appearing angry or pink for several months postop.Seek a Board-Certified Plastic Surgeon with additional training, experience, or comfort level with Oculoplastic Surgery to meet your facial rejuvenation goals.
