Going based on your side profile, I think a genioplasty with jawline contouring and submental liposuction would be beneficial. This would bring your chin forward and down to help balance out your profile, elongate and add more definition to your jawline, and reduce the skin and fat under your chin. Other things to consider would be a rhinoplasty revision, buccal fat removal, brow lift/repositioning, and a fat transfer/cheek implants.
