Question

I sweat quite a bit, like excessive in normal weather. While it sucks to be so sweaty, the worse is above my lips (creating a sweat mustache) and in my hair line, so everyone can see how sweaty I am. Everywhere else I can hide except for that. Any suggestions? I have heard of Botox to control the sweat, but I know it is controversial. What can I do?

Hello and thank you for your question.  Botox is well known to reduce sweat production, particularly in areas like the armpits, hands/feet or even the hairline.  These areas can handle a large amount of Botox so as to not interfere with muscle control and at the same time address excess sweating.The upper lip, on the other hand, is a muscle we use almost all day long and thus we need to be careful when injecting Botox in such an area. I often treat the upper lip with a small amount of Botox (2-6 units) to help reduce and prevent fine lines, which in turn, can also help reduce sweating.  It is a good idea to consult with an experienced injector who has treated those with hyperhydrosis in the past. Botox in the forehead can control sweat along your hairline if you have hyperhydrosis, which is medical condition in which you sweat profusely. The lip area is a little trickier, because too much Botox could interfere with your mouth’s function. However, we’ve treated many patients at our center for hyperhydrosis in the mustache area, and the treatment is very effective. You’ll need to find a board-certified dermatologist who’s an expert in the delicate lip area and the hairline with Botox.

Botox does block sweat. That much is true. However, your lip looks the way it does due to muscles that structure the lip.  The botox needed to block the sweat would also paralyze your upper lip. Don't do this to yourself.

I recommend getting a formal evaluation with a cosmetic dermatologist for hyperhidrosis. Botox works well for improving excessive sweating, however if you have significant amounts you may need oral treatments like glycopyrrolate. See an expert. Best, Dr. Emer

Botox is an approved treated for hyperhidrosis or excessive sweating particularly in the underarm or axillary area.  It will be effective in other areas of the body to prevent sweating and I have seen this especially when we treat the forehead area.  However, Botox in the upper lip will also effect the muscles in this area as well as the sweating issue.  I do use small doses of Botox for perioral lines (smoker's lines) so I would recommend having an experienced injector give a trial of similar small number of Botox units to see if it is effective for your sweating.

Botox is great for sweating. This is mostly done in the armpits for "hyperhidrosis", which means excessive sweating. It has also been described for the scalp and hands. That being said, I would not recommend it for upper lip sweating bc it will affect the muscles that control the fine movements of your lips. This could affect your smile and mouth movements.

Botox can paralyze the tiny muscles under each sweat gland which pump out sweat to prevent excessive sweating. This is particularly useful in the armpits and is well-studied and documented as a solution to sweating. It can also be done in the upper lip area, however the deeper muscle controlling the lips is also in the area. There is a high chance that targeting your sweat glands will also affect your ability to move your upper lip. If you wish to proceed, then only a very small amount should be placed in the area. You may only get improvement in the sweating rather than complete resolution, however it is less likely to affect your lip muscles adversely. Hope this helps!
