Would it be bad to smoke hookah at 3 months post op or better to wait longer or no longer smoke it at all?

If you smoke double apple you will be fine! All jokes aside you will be fine this far out. Of course as a doctor I have to say smoking affects your microcirculation so theoretically it will affect hair but there is no proof that smokers have a higher rate of being bald. Too many factors involved for smoking to actually affect your hair.
