How can I get rid of the perioral fat mound or whatever the fatty thing is called next to my mouth?

Do I need PDO threads or more filler? I am 38 and had one syringe of Voluma in each cheek, one unit of Botox in depressor angelis oris on each side and some Restalyne in the nasolabial folds.

Accutite is a radiofrequency which zaps the fat and tightens the skin to smooth out areas that need it on the face and it really works well.  Don't continue with temporary fixes when Accutite can come to the rescue.

Thank you for your question and photos.  It would be helpful to see your midface. Perhaps there is room for lifting the lower face with more filler in the mid face. But I can not determine until I see your entire face. The perioral area can also be improved with the unsightly part disguised with filler. After all this is done, if you still need more lifting, I would look into Mint Threads in the hands of an expert.

so what do you have is a classic part of mid facial ageing. As the midface falls, it leaves a tear trough deformity under the eye, it swings your cheek down towards the nasolabial fold and create this mound which is part of the marionette line complex. Volume high on the cheek can inflate the upper part of the cheek somewhat. In my experience trying to fill underneath this overhang does little to disguise the problem and certainly doesn’t address the anatomical ageing. My advice for patients like yourself is to seek a consultation regarding mid facelift. This is an excellent operation which addresses this problem as well as the descent of the cheek complex. Hello! I have recently over the last year had a lot of success injecting Kybella in that exact area to diminish that  fat pad. It will swell fairly aggressively for about 4-5 days, but after that, you should start seeing a nice result after 2 weeks. May need a second and third treatment for full treatment. Just one vial split between each side per treatment.
