Will placing a chin implant in a 45 degree angle cause more or less bone erosion?

Thanks for sharing.  A chin implant is designed to sit flush on the mandible (bone) and not at an angle. So, although your request sounds like it would solve your problem the implant can not reside in such a way.  In fact it most likely would slip or move out of position when placed as such.  Speak to a few board certified surgeons to evaluate your candidacy.

Chin implants are manufactured in a very large array of sizes and shapes, therefore a full set of photos are mandatory.  For many diagrams of the implants ,and B/A photos, please see the link below.  A  very small amount of bone resorption and remodeling does occur, but it is a non-issue

If you want combined vertical and horizontal elongation of the chin, you should get a custom chin implant made that is designed to do exactly that type off dimensional change. Trying to make a standard chin implant sit in a position for which it is not designed is a setup for a postoperative implant malposition issue.

The more pressure the implant puts on the bone the more bone erosion. The bigger the implant the more pressure it exerts on the bone so the more erosion. It is not a viable option to place the implant in the soft tissue.

i don't believe any research has been done indicating a difference in bone erosion based on angle of placement of the chin implant.  I think no matter the angle that a chin implant will cause bone erosion.  I actually favor using filler.  Good luck! DSR

  Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  Non smiling photos of your face from the front and side would help in the evaluation.  In my experience and humble opinion, the vast majority (almost 100%) of chin implant patients over the past 30 years have had a properly balanced face in the front, which is determined by "equal thirds" vertically: upper 1/3 from labella to frontal hairline; mid 1/3 from labella to base of the nose (columella/lip junction) and lower 1/3 from columella/lip junction to bottom of chin.  Your face most likely has the same equal third proportion which means the chin appears to be vertically short when it's horizontally weak as described below.  True bone erosion is rare and would not be affected by placing the implant at a 45 degree angle (this angle of placement would not correct a weak chin and would result in the implant wings being oriented in a tangential angle along the sides of the mouth creating a "joker grin or ridge". When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  Proper placement of a silastic chin implant adds "forward" projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty.  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  Hope this helps.
