I have dimpled and wavy cellulite on buttocks, thighs and lower back. I am 42 yrs of very fit woman. My objective is to reduce cellulite, even out irregularities due to previous liposuction on lower back and improve skin texture. Which of the following technique is the best (downtime or cost is NOT a concern to me). Profound RF, cellfina, cellulaze, vellashape, Thermage, Bellashape and Emtone. Can doctors please provide unbiased advise REGARDLESS of which machine they use?

Answer: Dimpled Cellulite: Consider Subcision; Wavy Cellulite: Consider Profhilo & Injectable Skin Boosters Of course without at least a series of HD photos, it is difficult to comment in any but generalities. But, what I can say is that most of the relatively expensive devices lack real hard science to support them and a far more backed by machine manufacturer marketing and sales hype. So, be certain to do your due diligence before agreeing to go this route. On the other hand, manual subcision with or without the additional use of a biostimulatory filler, such as dillute Radiesse, for elevating and smoothing depressions dimples of all kinds is relatively inexpensive and has stood the test of time. For wavy cellulite, superficial injections of Profhilo, a unique hyaluronic acid-based bioremodeling agent, followed by the use of a hyaluronic acid-based skin booster, such as Restylane Silk, Viscoderm Hydroboosters, Juvederm Volite, etc., can help to hydrate, smooth and tighten the overlying skin. Profhilo is unfortunately not yet available in the States nor are most skin boosters, but Restylane Silk can be used substituted for the purposes of "skin boostering," as can appropriately diluted Radiesse, in the same dilutions as used when rejuvenating a crepey, crinkly neck. Make sure that you seek consultation and treatment by a board certified aesthetic physician and not just anyone or medspas hawking their supposedly "next big thing" bells and whistles devices. I would recommend that you have a combination treatment of dilute Sculptra injections (not to give you a large butt, rather to smooth the texture of your skin.) We combine the Sculptra injection with a heat device, be it Profound RF, Venus Legacy, or Ultherapy. Emsculpt of your buttocks will also help by building muscle tone which helps take up the slack.
