Question

How do I know if I need more hyaluronidase to dissolve all fillers in my face, or if there are even any fillers left in my face to dissolve/? I do not know why I still have this dent like thing under my dam eyes I just don't feel like living anymore

Longterm filler problems include migration, edema, and nodularity. Filler migration and permanence is a very common problem because there are many different types of HA fillers on the market and each is specified for a specific part of the face. Use of highly cohesive fillers on thin skin may cause swelling and nodularity. If patients have filler migration or nodules, we recommend a combination approach of filler reversal with enzymes Hylenex and Hyaluronidase in different strengths to dissolve the filler. We may also need to add Kenalog to the mix in order to reduce fibrous tissue. If the filler does not dissolve, it either means the filler is not HA material or we need more enzyme. The HA reversal can be combined with other lasers and skin treatments at the same time, and it usually (usually) does not affect adjacent tissue.

If you have reversed the filers 2-3 times they are likely gone.  Based on photos you have normal aging, sagging and need the proper treatment options.  I suggest you see an expert.  Best, Dr. Emer.

Most often, people need 1-3 treatments of hyaluronidase to remove fillers - for non-arterial issues. That's a general range for a "normal" amount of two syringes or so. To see if you "still have fillers" you should consult your pre photos. That would be the best way to determine it. Fillers are generally soft so unless you can feel them under the skin, you won't necessarily know they are there. As far as the dent, that appears to be general volume loss in the face. I don't see filler there. So best to consult your pre photos to see what you were trying to correct and what was there before. "This answer has been solicited without seeing this patient and cannot be held as true medical advice, but only opinion.
