More specifically I am wondering if my midface concavity can be corrected with rhinoplasty alone to stretch the skin and create a more proportionate nasolabial angle, and is the degree of my desired tip projection and rotation realistic Thanks.

Answer: Rhinoplasty I don't think a premaxilary implant will be of any benefit whatsoever. It is not clear what you need from the limited photos you submitted. You should go on a few different consults with experienced rhinoplasty surgeons to determine your options.

Answer: Pre-maxillary implant with Asian rhinoplasty This is a good question to ask. It's not talked about much but midface or maxillary weakness or retroposition as we sometimes describe it can limit the overall outcome and I guess I would say "presentation" of rhinoplasty results on the Asian face. One of the best rhinoplasty surgeons in America, Dr. Dean Toriumi, has a very well-known before-and-after photo of the Southeast Asian woman with a flat midface and nasal features typical of her background. In the after photo there is detailed conversations and descriptions about augmentation of the bridge and tip work with cartilage but what is not always mentioned is she also had premaxillary augmentation. Augmentation of the premaxillary area in the Asian face in the context of a rhinoplasty sometimes is needed to help a person reach their goals. In your photographs it is a little difficult to say but my general analysis is that your goals are probably obtainable without premaxillary augmentation. I cannot say for sure without a physical exam but I think you're at least on the right track and probably have been to some good consultations because many patients never even know that this is a potential component of their surgery.Chase Lay, MDAsian facial plastics specialist
