I have always had these, even as a kid. My mom has them too. It shows worse in different lighting but you can feel the crease. I have been told everything from fat-graph, to mini-face lift (I just turned 30), to juvederm voluma or restylane lyft. I want something long lasting (permanent hopefully) to fill these creases, but if fillers are the best way to go I am open to that too. I would like to know what these are officially called.
Answer: The line you are pointing out is not the tear trough. This line is the mid cheek groove. It is precisely a crease but is is not a separation from the "cheek muscle." There are several ligament systems that meet here and contribute to the these lines. I strongly advise against grafted fat. Grafted fat causes scar tissue to develop. The cheek here has many layers that need to glide against each other. After fat grating any number of problems arise including the loss of this glide function. That means that every time you smile, the lines in the cheek will not be right ever again. You are pretty young for that type of change. I strongly recommend filling this line with hyaluronic acid filler. The service will last about 1 year. Do not let anyone inject a so-called smooth filler, for example, but not limited to Voluma, Juvederm, Belotero, and/or Restylane Silk. These products are smooth because after the injection they shift around. This smooth character means that you are less likely to have lumps and bumps. Unfortunately, the product shift means that your line will not necessarily stay filled up. The solution is to use the right filler. I prefer Restylane, but Restylane Lyft is also usable in this area. There is no substitute for working with physicians who know what they are doing and are honest about your issues.

Answer: Improving Under Eye/Cheekbone Indentation -- Fillers (Belotero, Volbella, Restylane Silk, Voluma), Fraxel, Microneedling/PRP A combination approach will give the most improvement. Fillers, lasers, and microneedling/PRP can improve under the eyes. My patients try to get non surgical treatments to avoid surgery. We use PRP and stem cells there often. Answer: Facial groove Thank you for your photos. It appears you have a tear trough deformity (crease along nose and lower eyelid which extends into a midcheek groove. The midcheek groove becomes more noticeable when we age and is most noticeable in certain lighting or from an angle where you see a small indentation in the curve of your cheek. For the midcheek grove, I like to use a strong volumizing filler like VOLUMA in two different layers. There is no downtime and it lasts a long time (1-2 years) and is reversible if necessary. I also like to use a combination peel to thicken some of the dermis that has thinned in that area. Fat grafting is more long-lasting, although it is more difficult to control exactly how much stays in each cheek and comes with a greater downtime. This occurs due to the natural again process or genetic aging. There are several options to correct this problem. Fillers are one solution and there are several on the market to choose from that would correct this problem. These include Juviderm, Voluma, Radiesse, and even sculptra. The injector will have their preference. Any of these will work but there are preferred fillers for this area. This would include Radiesse or Voluma. These have more volume associated with the product. This area needs volume to correct the problem. The other option is Fat Transfer. This would be my recommendation for this area because it will give you the Volume that is needed. This is a permanent solution and long term. It will age with you and would not have to be repeated like fillers. The filler option needs to be repeated. Fat Transfer is long term and permanent.On all my facelift patients, I fill this area in with Fat Transfer at the same time.
