I had a rhinoplasty to correct a deviated septum and to reshape my n’ose tip which was dropping and bulbous. After almost 8 years I’m satisfied with the length nd width of my nose however the nose tip is becoming bulbous again. I asked my surgeon if I could do a revision but he does’t agree and suggested that I take a medicine called “NETLOK”for a couple of months! I’m skeptical that this pill would cause a lasting change if any at all, besides it has several side effects.. advice please?
Answer: Tip Changes Hello,Although skin can get thicker, this rarely causes a visual change. I suspect your tip changes are due to changes in cartilage position and loss of support of the tip complex from the underlying septum. 'NETLOK' appears to be a messaging app! Regardless, medication is not the answer. You appear to be an ideal candidate for a revision rhinoplasty, using structural techniques to give you a long lasting result. Go visit a few ABPS certified/ASAPS member surgeons who specialize in revision rhinoplasty. Best of luck!

Answer: Tip size after rhinoplasty Thanks for your photos and for sharing your concerns. 8 years means your nose is essentially completely healed. A physical examination would allow a surgeon to determine if your concerns with your nasal tip are the result of cartilage, scar tissue, swelling, or something else. This far out, it is safe and reasonable to revise if you are bothered by it. A medication is extremely unlikely to create any significant change, and even less likely any long lasting change. Revision rhinoplasty is unique from primary rhinoplasty, and you should seek treatment from a board certified plastic or facial plastic surgeon with experience in revision nasal surgery. Warm regards.

Answer: Rhinoplasty Hello - it would be hard to imagine your nose becoming bigger with time. Perhaps if you showed a side by side comparison it would help. Best to seek consultation with a board certified plastic surgeon that is comfortable with re-do nasal surgery. The video attached will explain why. The web reference will take you to a page where you can se re-do nasal procedures. Best, Michael Kulick MD

Answer: You could consider a revision rhinoplasty... Your tip cartilages are very strong. They make the tip wide, and they make the tip stand out when you smile. Rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself very confident in your surgeon's skills. For most noses, it's much better to not have surgery than to have inexpertly-performed surgery. I have written an essay on how to stay out of trouble while searching for a rhinoplasty surgeon, and how to tell whether your previous surgeon should be performing your revision. Your nose is also a good example of why computer imaging is mandatory in rhinoplasty. You need to know exactly what the surgeon is planning to accomplish -- what features he thinks he can change, and by how much he thinks he can change them. I always make morphs for my prospective rhinoplasty patients. When you see the surgeon's goals, you'll know whether he has an eye for an attractive nose, and whether he shares your opinion of what constitutes an attractive nose. You'll also know whether the changes he proposes are enough to be meaningful to you, and whether he understands your wishes enough to address all of your priorities. How would you ever get that figured out without the morphs?! But remember, you're not the surgeon for his skills with the computer. The doctor must then show you his before and after photos to prove that he can actually accomplish what he draws on the computer. In your case in particular, you need to find a surgeon who understands how to narrow, elevate, and deproject the tip cartilages, and how much change is needed for an attractive nose. The medicine will do nothing. Bad idea.

Answer: Rhinoplasty Hello and thank you for your question. Eight years after rhinoplasty, the bulbous nature of your nasal tip will not change and I never recommend any such medications to my patients. If you are not satisfied with the width of your nasal tip at this point, then a revision rhinoplasty would be the best route in my opinion. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. You should be able to view numerous pictures of actual patients with noses similar to yours performed by your surgeon. Best wishes and good luck. Richard G.
