Hello, based on my experience with Truform III implants, I haven't noticed an increased incidence of the snoopy/waterfall deformity. This shape can result from any saline or silicone breast implant placed in a subpectoral pocket. I also haven't noticed they take longer to drop. They are certainly firmer than Truform I or II implants, most patients don't complain that they feel excessively firm.
If your own breast tissue drops and the implant does not, this will cause the waterfall deformity. this can happen with any implant, but may be more noticeable with the higher profile implants. If this happens, you need a lift. Higher profile implants can take longer to settle, especially those under the muscle.
If you lose 50 pounds, your body will be slimmer and your breasts will be more noticeable. If you have much fat in your breasts, they will be smaller with weight loss. We all look better closer to our ideal weight.
This repeat operation would likely not need any mesh and any type of new generation silicone implant (any company eg. Mentor, Allergan, Sientra). If that picture is your final outcome...I would not recommend messing with this kind of a result unless you really really want to change.

The risk is still low, but textured have a shorter lifespan than smooth. Also, the textured are not as soft and do not move as much as the smooth. there are some advantages and some disadvantages to this. Good luck with your surgery.
