I've had several Fraxel dual treatments to some areas on my body for hyper pigmentation. I use spf 30 and above sunscreen with mineral ingredients (titanium and zinc oxide) daily and frequently. Can these freckles come back over time? Are my cells permanently damaged?

Sunscreen is not required indoors after Fraxel treatment. I do recommend my patients wear sunscreen anytime they are outdoors, and especially in the weeks post-Fraxel treatment I do encourage my patients to use plenty of sunscreen on the newly revealed and healing skin.
