Question

I had the surgery last December on my left lid and if I'm honest don't see much difference in the ptosis, and I actually seem to have developed a new problem. I have a extra ridge under my eyebrow and in my eyelid. It's irritating and looks like a slight deformity now. The surgery was going to be on the muscle but the surgeon on the day said it would be better for me to have the eyelid stitched instead to get a better result. However it doesn't seem to have worked and I have another problem!

The asymmetry and "extra" skin is likely due to mainly volume/fat loss, which means filler injection can help. See link below for some examples. Best to see an oculoplastic specialist for evaluation.

However, ptosis is humbling business.  I believe that you are describing an anterior levator resection ptosis surgery.  That is the correct operation.  However you it would have been helpful if the surgery opened the eye more and if the surgeon had teased forward anterior orbital fat to help full the upper eyelid fold.  That is a nonstandard detail.  For now this eyelid needs to heal for at least 6 months.  Finding a surgeon with the skills to further repair this eyelid will be challenging.  Many do not like revising prior ptosis surgery because the believe all the planes are scarred.  However experience has shown that many of these eyelids can be revised.  Study the attached video.

Hi there, it's difficult to tell which eyelid you had surgery on with the photo but it seems like you have some volume loss in one of the eyelids, resulting in an "extra ridge." Upper lid fillers could certainly be an option, best advice is to seek the opinion of an eyelid expert. Best of luck...
