Infini RF - scars did not respond - is it the treatment parameters? (Photo)

Dear docs, I recently had two Infini RF treatments for my acne scars (attached pics don't show them that well). Unfortunately, I have had no visible improvement. I asked the doc for the treatment parameters. Perhaps one of you doctors has some suggestions as to how the settings could be adjusted? Your skin looks better to me but you still have active acne that needs treatment.  I suggest a combination approach of microneedling RF combined with erbium or microneedling/PRP.  you will need more than 2 treatments to get improvements and will need at least 2-3 times a year as maintenance forever.  Best, Dr. Emer.

The settings you mentioned are more superficial than I use for acne. To get the best results, multiple passes at 2.5 to 3mm are generally indicated. 4 treatments will sometimes be necessary. Acne is always difficult to treat and other modalities may be necessary to enhance the overall results.

The settings that you reported are not generally adequate for treating acne scars.  To reduce acne scarring, the needle depths typically recommended are deeper than those used for your treatment, up to 2.5mm-3mm, for example.  The pulse width (the duration of time, in milliseconds, that the radiofrequency energy is delivered to your skin), is also shorter than typically used to treat acne scars in my experience.  In my practice, we use longer pulse widths, 120ms-180ms, and lower energies, level 3-4, when targeting acne scars.  We see significant improvement using these parameters to treat acne scarring.  You may benefit from a second opinion from a Board Certified dermatologist experienced in RF microneedling.

I see improvement of the skin. INFINI has very good results in acne scarring.The setting of device are now designed as low med and high and the treatment settings depends on the condition and the area to be treated. To help the active lesions a combination of chemicals peels and Retin A is a good option. For scars PRP and Bellafill are good. LM

These are not standard settings. If I'm reading right, you had some at 90 and 30 ms and at 0.5 and 0.1mm?  I usually don't go under 100ms, and never at depths of 0.5 and 0.1mm. The US should have all machines switched over to the new user interface that now has built in low, med, and high settings. Depending on the person, settings should be medium for the deeper settings and the more superficial settings at low. I go from 2.5-1.5 on the cheeks in lighter skin patients and 2.5-2mm on darker skin. You need to have better control of your acne too. This is partially treated with the Infini, however Retin A, chemical peels, and oral antibiotics might be needed. I have found the Infini to be the best treatment for acne scars, well ahead of lasers.

Firstly, you have active acne. This needs to be treated. INFINI RF is a brilliant device, however it is how one uses it that gives the result. This device is one of the most complex but most adjustable RF devices on the market- that is why I love it. Knowing the correct settings is vitally important as this gives the best results. The pulse duration needs to be longer, the power settings needs to be higher, and the depth adjusted according to the kick back spring rate of this system and level of your scars. Kind regards, Dr Davin Lim.
