I got 5 weeks ago infini rf on my acne scars at settings below with NO RESULTS. What would you recommend my settings be for next time? I was thinking about highering the level to 13 and changing the pulse duration at 400ms. (I have mild acne scars and large pores on the nose). That be great if any doctors can recommend me the correct depth, passes, level, and pulse duration. 1st Pass 3.0 Level 3/300ms 2nd pass 2.5 Level 3/200ms 3rd pass 2.0 level 3/120ms

Infini is a popular treatment, but I prefer Intensif because it uses non-insulated needles and provides more robust results. Keep in mind, acne scarring requires a combination approach with microneedling with prp, lasers, subcision and fillers. It will take a series of treatments. also you will need 3-6 treatments to get outcomes with these devices unless they are combined with ablative lasers. See an expert. Best, Dr. Emer.

Answer: Acne scar treatment with Infini - energy levels First off, deeper scars need other treatment in addition to Infini. Bellafill is needed for atrophic scars that are deep. Research has shown that despite what seems correct, higher energy levels aren't as effective as lower energy levels in delivering heat to the skin with the Infini. High treatment levels lead to desiccation of the tissue because it heats up too quickly. This creates a coagulum around the needles and the heat stops there and doesn't flow into the tissues. The best settings are low TL (2-5) and long pulse durations 300-400. All the units in the US should be upgraded to the new user interface that has preset (low, medium, and high) settings for the different areas of treatment. All these settings are fairly aggressive. I have rarely used the high settings because of prolonged downtime and increased risks. The deeper the energy is placed, the safer it is to place higher energies. If doing 3 passes, I use med, med, and low for acne scars, with the most superficial pulses being the low. It also has to do with skin types. Pulses should not be more superficial than 2mm with these settings in type 4 or greater skin. Even if the pulses are placed at 2mm, energy/heat can spread to the surface so caution is key. After each pass, the skin needs to be evaluated and see if it can tolerate more energy. Depths are appropriate but I might to 2.5, 2.25, 2.0. Also, it is not true that Intensif leads to more robust results. The uninsulated needles are limited in their energy delivery or they would injure the epidermis too much and lead to wound care, scarring, and pigmentation risks. The power behind Infini is essentially well beyond what the skin can tolerate safely. In addition, if uninsulation is desired, placement of the needles at a depth of 0.5mm will give an ablative wound similar to Intensif or other uninsulated devices, but it is not recommended.

Your specialist can adjust the parameters depending on the depth of your pathology, acne scars need a longer pulse duration, especially with deeper scars, this is balanced between the energy level. Too high an energy level will char the insulated tips. We really do need to see, touch - visualise your scars. Also if you have tethered scars, the spring on the Infini (even at 3.5mm) will 'kick back' so this has to be adjusted according. Your Specialist will know how to adjust all the settings. I often combine this with fillers at a later stage, but almost always with PRP in an over and under setting for quicker recovery times.
