7/1/24 had upper bleph with endoscopic eyebrow lift Went back and pics look same! Question should I have a direct eyebrow lift to get rid of extra skin on outer upper eyelid or am I missing something here. I need direction, I spoke w original surgeon and he said can do direct brow lift. Will this resolve my issue of sagging skin?

Droopy forehead and eyebrows you had were candidate for a forehead and eyebrows lift; this can be better done by means of short incisions in the scalp or by endoscopy, I always perform and recommend short incisions since endoscopy does not allow good fixation of the lift and is short lasting or fails, like in your case, you don't have any excess of skin but just the tail of the eyebrow (and the mid end) are still droopy and saggy; this short incisions forhead lift to raise the brows can be dan subperiosteal or supraperiosteal, I recommend and I do supraperiosteal to avoid the chronic swelling associated with the subperiosteal lift; this can be done standard (only forehead and eyebrows) or extended, I do the extended version which covers the cheekbone area, the mid cheek (both lifted) and a smoothening of the nasolabial folds.

Threads or thread lifts are a de facto technical scam, if not a source of nerve damage, deformities, etc. Direct brow lift is a mid-XXth century procedures, should not be even considered by a surgeon nowadays (terrible scarring); coronal or trychophitic/hairline forehead lifts are source of bad scars, visible baldness, numbness and short lasting effects.

I don't see much of a difference between your before and afters. I think a temporal brow lift with a revision of the upper bleph would be a great benefit for you.

It appears that your entire brow is at or below the orbital bone. Two options include a direct browlift or a subcutaneous browlift. Both would provide a permanent result as opposed to an endoscopic left, which does sometimes relapse. In women, I prefer the subcutaneous browlift to hide the scar in the hairline. In some patients, the direct browlift approach leaves a visible scar for a long time, so I often perform this procedure in men with dark complexions.
