From your pictures, I would be conservative and just watch these areas heal. I would not rush to surgery for revisions. I would allow your body to heal for at least six months before any revisions are scheduled.  I should be mention and stress that my opinions are solely based on your history provided and pictures, not a thorough physical examination. I would suggest you stay with your plastic surgeon.   Since your plastic surgeon knows you best, I would follow his or her suggestions and not rely on a platform like this for specific medical or plastic surgery advice. I would suggest you to follow your surgeon's instructions and consult him in case you are concerned. your outcome is really good.  Fat necrosis in of of itself, is not anything you should have to worry about.  But it should slowly get smaller with time.  If your surgeon is seeing your regularly, that is all you should need until this becomes a non-issue for you.  But trust your surgeon to keep doing the right things for you as he/she shares your goal for the best outcomes possible.

Thanks for the photos and the question. Overall, your results look very good. Your surgeon is the best judge of what may be going on. Fat necrosis is not uncommon in  massive weight loss patients and patience is important. Sometimes, if the fat necrosis is nodular and painful, it needs to be removed, but sometimes it softens over time. In the absence of infection, I'd probably advise patience but I did not do your surgery nor examine you.
