I have an inherited shallow orbit and large, prominent eyes as well as negative vector/deficient midface. My eyes look oversized, sad, and tired. What are my options for correcting the appearance of my eyes? I have read that lower bleph can result in further retraction. My interest is solely to find something which supports my lower lids. Would addressing the cheeks help prop up the lids? I am 27 and eager to find a more permanent solution than fillers.

Answer: Lower eyelid retraction Thank you for the question, a photo would help in terms of specific recommendations but in general for lower eyelid retraction there are two major ways to address the problem. First, as you mentioned negative vector and mid face deficiency, one option to address lower eyelid retraction is with what is called an orbital onlay implant, essentially this is a small synthetic implant that is placed on the orbital rim and the front surface of the maxillary bone to help push the lower eyelid into a higher position. The other option for lower eyelid retraction would be with the placement of a spacer graft (hard palate, Enduragen, Alloderm) inside the lower eyelid once the lower eyelid retractors had been released. I would recommend finding a local Oculoplastic surgeon to discuss your surgical options.
