Is it possible to remove my dimples? (Photo)

Hello, I have unusual dimples that appear due to the muscle movement around my mouth. They are hard to capture in a photo, but very evident in person and several doctors have agreed they are prominent as well. I have tried botox and I guess, due to how deep they are, the effect never fully worked and did not even last a week. I am wondering if removal by any means is possible, they make me deeply unhappy. Dimples caused by muscle movement around the mouth can be challenging to treat, especially when they are deep and dynamic, as you described. These types of dimples are usually due to strong or uniquely positioned facial muscles that pull the skin inward during expression, rather than just a surface issue.

Botox is often the first treatment option, but it does not always work well in this area. The muscles around the mouth are complex and essential for normal function, so dosing has to be conservative. In some cases, this leads to minimal improvement or very short duration, which seems consistent with your experience.

If Botox has not been effective, there are still options. Dermal filler can sometimes help by adding volume beneath the dimple to soften its appearance, although it will not stop the muscle movement entirely. This can make the dimples less noticeable rather than fully eliminating them.

For more definitive treatment, surgical approaches may be considered. These involve releasing or modifying the muscle attachments that create the dimple. This is more invasive but can provide a longer-lasting improvement in carefully selected patients.

Because this is a nuanced area of facial anatomy, the best approach depends on identifying whether the dimpling is primarily due to muscle pull, skin tethering, or both.
