I am 25 years old and i have had a problem on my butt that was caused by injections in my childhood that left big holes... I am 74kgs, black in color.
Answer: First, the fibrotic area needs to be released Hello, Due to fat necrosis caused by the injection in your buttocks, significant fibrotic tissue has developed in the area. For this reason, if fat is injected into the sunken areas, the fat will spread toward the surrounding areas rather than the sunken region itself. First, the fibrotic area needs to be released with a V dissector or surgically freed. After healing, fat injection can be performed. However, since you have extensive areas of depression, this may require multiple sessions rather than a single one.

Answer: Treatment for holes on butt from injections Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made. Because these “holes” are actually severe dense scar tissue adhesions as a result of loss of the intervening fatty tissue between the skin and muscles, I have found that the only reliable long-term option is to try to pop them out with Stanton Anatomic®️ buttock implants. Unfortunately, and again because of the severe nature of scarring involved, BBL fat grafting and fillers will usually fail in these cases. Please note that an extremely few number of board certified plastic surgeons specialize in buttock or hip implants, so you may need to travel far and away from home to find this particular surgeon. Answer: Buttock Scars One option is to excise the deep scars, undermining any areas that are tethered and reclosing for an improved contour. Another option is to use tissue expanders to stretch the skin and then excise the scars and reclose. Fat grafting will not correct the contour.

The underlying ideology is most likely scar tissue that tethering the skin down to the muscle layer. Releasing the scar tissue by itself may give some temporary improvement, but the contour is likely to return. Releasing scar tissue in conjunction with augmenting the area with grafted fat may give better results. It’s not an easy condition to correct. Some improvements can probably be obtained and it’s possible that it may require a few treatments to get further improvements. Simply adding volume by injecting fillers is unlikely to improve your outcome. If a partial improvement is sufficient to have surgery, then start consulting with plastic surgeons in your community to see what they have to say. Recognize that this is a challenging condition to treat go into it, knowing that the results may be less than ideal, but partial improvement should be possible. Best, Mats Hagstrom MD
